Application form: Disaster Welfare Support Volunteer

As part of the application process, you'll need:

• The details of an emergency contact person.
• The details for two people that we can phone to undertake a character reference check.
  Your referees should not:
  
  • be related to you or part of your extended family
  • be your partner or spouse
  • live at the same address as you.
• If you are aged 16 or 17, you will also need the details of a parent/guardian.

Parent/guardian details are needed if you are under 18.

Please provide the details of two people we can contact.

Reminder: Your referees should not be related to you or part of your extended family, nor be your partner or spouse, nor live at the same address as you. For example, they could be your employer, professor, friend, or someone who knows you well.

Please note this information may be provided to our vetting panel who make decisions about volunteer eligibility.
